Product Information Form

Sending your Product Information Form digitally will help expedite the initial check-in and startup phases of your testing AND will speed up the report production after the testing is completed. Please fill out this form and return it to Criterion Technology at least two days before your scheduled test to confirm your product information and your testing needs. If there are any questions regarding the information required in this form or if you are in doubt about what tests are required for your product, please contact us either by phone or email.

The Product Information Form is available in .doc format, .pdf format or plain text.

Instructions

1. Select one of the above versions of the Product Information Form.
2. Either open it or save the file to your computer (right-click->Save Target As...).
3. Fill out the form either electronicly or on a printed copy.
4. Send the completed form to Criterion Technology.
